When you and your family members choose to participate in the cHIE, it can help your family in several ways:
•You can have peace of mind from knowing that all of your doctors will know the right way to take care of you and your family and will not give you treatments that might not work for you. And you will not have to wait for one doctor to send your records to another doctor. If you give permission, your records can be available in the cHIE right when they are needed.
•If you help your loved ones get medical care, you will not have to worry about remembering medical information for all of them. The cHIE can keep track of it.
•You can save time and money by not having to repeat tests that you’ve recently had or try treatments that didn’t work for you in the past.
•You can be sure that your family’s medical information is safe. Information in the cHIE is kept secure according to federal law. Only healthcare professionals can have access to your information in the cHIE and only to give you treatment.
You can choose how you want to participate in the cHIE. To make your choice, get a Patient Consent Form from a participating cHIE doctor or print one from: http://www.mychie.org/patients/.
